关于 JUUL 电子烟集体诉讼

如果你在 2022 年 12 月 7 日之前，购买过 JUUL 电子烟，可以参加集体诉讼，获取总金额是 $255 M 的赔偿。原因是 JUUL 隐瞒了电子烟的成瘾性以及给未成年人非法出售了电子烟。当然，JUUL 是否认任何有罪行为，只是同意提供赔偿。

另外，如果你购买金额低于 $300，是不需要购买的证据的。超过 $300 就需要提供了。

重要日期

重要的时间就是提交的截止日期是 2023 年 7 月 14 日，如果你有异议也需要在这个日期内 opt out 或者给法院写信提出反对。

如果你收到过有关这个集体诉讼的 claim number，直接填入就行，建议直接选择网上填写，没必要下载表格弄半天还要邮寄。

如果你没有收到，而且确实购买过 JUUL 的话，你可以选择没收到 claim number，或者在下面这个链接直接填写新的 claim

提交基本信息后，你需要填写购买电子烟的时间

以及购买电子烟的数量

如果申报购买的金额超过 $300 是需要提供购物凭证的，比如小票、信用卡账单、网上订单的 confirmation page 等 

Documentation: At the end of this Claim Form, supporting documentation will be requested if you elect to submit your purchase information via documentation OR if the claimed quantity of purchases is greater than $300. Acceptable documentation includes receipts, bank/credit card statements, and/or pictures of online confirmation page (s) .

“Self-prepared” documentation such as handwritten explanations are, by themselves, insufficient to receive payment.If you do not have your documentation available at this time, you should still submit the claim form. After your claim is reviewed, you will be given instructions by email as to how to upload your documentation. Without documentation your claim will be limited to $300.
